How to Start the Application for AZ Long Term Care

There are many different ways of starting an application for AZ long term care. Let me tell you that if you have any questions at all, feel free to give us a call. First, it is important to have all of the supporting documents for the application. This includes medical records, bank account statements and statements of other asset ownership. The qualification will be based upon medical and financial need. Once you have all of the supporting documents, it is time to begin an application for AZ long term care.

Obtaining an Application

After making a call in, the application will be mailed to an address of your choosing. Once the application is filled out and returned, you will be set up with both a financial evaluator and a medical professional who will come out to do the medical evaluation. Make sure that you have everything documented for the medical evaluation. Many times the patients don’t want to disclose the true nature of their health and that is why it’s important to have doctor documentation as well to prevent a disqualification.

How Do I Get Qualified Medically?

There are 60 points total and you receive points for a qualifying diagnoses. You also receive points for not being able to perform your ADLs. These include not being able to cook, clean, bathe, take medications, and get dressed. The risk for falling gets you points along with a dementia or Alzheimer’s diagnoses.
